Sizing Form for ___________________(Customer's name). Each customer must complete a separate sizing form. Please take the following measurements using a cloth measuring tape. If you wear body armor, please take measurements with your vest on. Uniforms are all custom made to fit the individual's exact measurements (Refer to picture diagram below).
_____ 1. NECK - Measure around base of neck.
_____ 2. SHOULDERS - With arms straight down at sides, measure all
the way around the fullest part of shoulders.
_____ 3. CHEST with VEST - Measure just under arms around fullest
part of chest.
_____ 4. SLEEVE - Hold arm up parallel to ground, bend elbow 90
degrees, then measure from base of neck to wrist, along
outside edge of arm.
_____ 5. CENTER BACK - Measure from base of neck to center of
belt line.
_____ 6. WAIST - Measure around waist. Please use your actual
measurement NOT your pants size.
_____ 7. HIPS - Measure around fullest part of buttocks or hips,
whichever is larger.
_____ 8. THIGH - Measure around fullest part of upper thigh.
_____ 9. INSEAM - Measure from crotch to point on leg where the
pant is to stop.
_____ 10. CALF - Measure around fullest part of calf.
_____ 11. HEIGHT
_____ 12. WEIGHT

The numbers shown below on the diagram coincide with the numbered measurement decriptions listed above. It is critical that you use a cloth measuring tape to take all measurements exactly as we've described above. Custom uniform orders are final; no refunds or exchanges are allowed.
